Note on Form B44A (Optional Signature Page): Form B44 (Settlement Application) references an optional B44A signature page. This is not a separate numbered form but an annexure that may be created as needed when the number of primary parties exceeds the space on Form B44. Any sheet listing party names with signature lines and dates is sufficient.
